First Steps

Thank you for choosing My Foot Rx as your orthotics and specialty shoe work provider. We are committed to a successful professional relationship with each of our patients. It is important for you to have a clear understanding of our patient financial policy.
We appreciate your prompt payment for our professional orthotics services. Please ask if you have questions about our policies and fees or patient responsibilities.
Warranty
Shoes can be returned if unworn for exchange for 30 days. Defective items can be returned up to 6 months for repair or replacement.
Custom items are not returnable but will be adjusted upon patient request for six months from the date of delivery. Any items or products that are abused, damaged, or worn past adjustable conditions will need to be replaced at patients expense.
Insurance
My Foot Rx accepts Medicare and most major insurance plans. Please present your insurance card each time you visit. Co-payment is due at the time of check-in.
Please contact your insurance company or employer with any questions about covered services. Your insurance company will make the final determination of your eligibility and benefits.
Medicare Reimbursement
Many patients are eligible for coverage of therapeutic shoes under Medicare. We can help you navigate the steps to receive Medicare reimbursement for your customized orthotics products. Contact us today with questions about our services or to schedule an appointment!

Recent changes to the Medicare Therapeutic Shoe Policy (Article A47129) now require more complete medical documentation. Please PRINT and COMPLETE the two forms below to be filled out by the physician who manages your diabetes. They are required for your initial evaluation with My Foot Rx.

During your visit, Bob McRoberts will provide an estimate of costs and will answer your questions. Self-pay pations will be discounted 5% at request.
